Form 1095-C Availability Notice

In accordance with Internal Revenue Code Sections 6055 and 6056, Chelan-Douglas Child Services Association (CDCSA) is providing this notice regarding the availability of Form 1095-C.
Form 1095-C contains information about offers of employer-sponsored health coverage made to eligible employees during the applicable calendar year.
Beginning March 2, 2026, individuals may obtain a copy of their Form 1095-C upon request.

Requests should include:
· Full legal name
· Current mailing address
· The calendar year requested
· A telephone number or email address for follow-up if clarification is needed
A copy of the requested Form 1095-C will be furnished within 30 days of receipt of the request, or by January 31 of the year following the calendar year to which the form relates, whichever is later.
This notice will remain available on this website through October 15 of the year following the reporting year.
